A brilliantly comprehensive and innovate web audio script, Otto can help you set up a user-powered internet radio station. It can also manage users, supports user uploads and has loads of other features to player with. It's written in Perl and requires Perl 5, MySQL, the DBI and DBD::MySQL Perl modules (plus a few others), and Apache - mod_perl is optional. mpg123 or IceCast are used to play or stream your music.
A mp3 database for apache/mysql with id3v2 and advanced browsing support. Features: MP3 album and file Catalogization using ID3v1,v2 (for track info) and INI files (for album info); Complete tracking of when albums and single mp3s are added to the archive; Web interface with all kinds of sorting and serching options, playlist management and much more. The files haven't been updated since 2004 but it looks comprehensive and has a hilarious website. The forum seems just about alive too.
Streamline is a streaming media server written in php. It is highly configurable and utilizes customizable visual themes. Installation is simple and includes a web based wizard. Other features include password authentication, and media tag decoding.

NetAMP is a fast and lightweight system comprising of a few PHP scripts and a MySQL database. You place all mp3 in a specific directory and their artist and title information can be retrieved by NetAMP in a variety of ways. This culminates in a searchable, downloadable index of your mp3s. Looks very scalable - no news on the site since the turn of the century though :).
Webbased Music Jukebox "is a PHP based jukebox for playing MP3 music straight from your Linux server. It creates a database of all your MP3 files with detailed header information(ID3). The jukebox can play in random, single and playlist mode." That's all the information included on this systems SourceForge page. It seems like it'll be similar to a number of other web apps on mentioned on this site, like NetAMP, however. Might be of use to somebody - no updates since 2003 though.
Another mp3 jukebox/index system. Tunez is a good-looking, clean interface with a few cool features plus Icecast support.
